• Membership card Must be present at time services are requested in order to receive benefits.

• Membership expires 1 year from purchase date

• Item's discounted by the week are not eligible for stacking (you may not use more than 1 per week.  If a week is missed, it will be skipped)

• Member name on card must match the person using the card

• Families wishing to use member benefits for more than one person must have all applicable cards present
